The Seminole Tribe of Florida Health Centers, a branch of the Department of Health and Human Services encompasses five Health Centers located on the Seminole Reservations in South Florida. The STOF Health Centers are staffed with highly trained physicians, nurse practitioners, nurses, and other healthcare professionals who provide health care for our patients throughout the life spectrum. Our staff in Tampa location provide care coordination, health liaison functions, and community outreach. Care is provided through integration and collaboration with members of the Department of Health and Human Services health staff, other tribal departments, and the tribal community.
The health team collectively, provides quality services across the healthcare continuum to its members and other eligible Native Americans utilizing the Patient Centered Medical Home Model. A care delivery model focusing on a personalized, patient – provider team approach to health care access and delivery through empowerment. Health screening, prevention, education, early intervention, acute, post-acute, and best practice chronic disease management are strategies implemented in support of our mission. It is our goal to ensure comprehensive, coordinated, quality care to improve outcomes and the attainment of each member’s optimum level of wellness.
The Seminole Health Department staff will provide quality healthcare and promote wellness within the communities to ensure that all clients reach their health potential.
