Welcome to Case Management & Quality​

The Case Management and Quality Improvement Department believes in a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy to meet the needs of individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes. When an individual reaches the optimum level of wellness and functional capability, everybody benefits. 

The case management team consists of Registered Nurses who work together with Social Workers, Psychologists, and other staff members of Health and Human Services to provide advice and education to our Tribal members to reach their health care goals. Case managers help identify the kind of help needed and assist in accessing the appropriate care at the right ​time and place. Case managers work with each Tribal member individually to create a personalized plan for treatment or recover. ​
